Do I need a Form W-9 from Spectrum Enterprise?

All Spectrum Enterprise services are provided by Charter Communications Holdings, LLC through one or more affiliated disregarded members. Annually, all Spectrum Enterprise clients who have paid us $600 or more during the year are required by the IRS to prepare and provide us with a Form 1099.
